"In Honor Of" Contributions
Recommended levels of In Honor of Donations are $150, $250, $350, $500 or more. In Honor Of Contributions can also be mailed. All In Honor of Donations are fully deductible.

Waivers
Young Adult guests attending the Presentation Ball under the age of 21 must check-in at the Young Adult Check-In to recieve their ticket and either sign an Alcohol Waiver on already have one on file. Young Adult guests over the age of 21 must check-in and secure a wrist band in order to consme alcohol.
Young Adult Check-In will be located in Hart Artrium on the Lower Lobby of the Morton H. Meyerson Symphony Center. Print sign and return Alcohol Waiver Form below:

Covid Protocol
Due to public health concerns related to COVID-19 spread in a communal setting such as at the Presentation Ball at the Morton H. Meyerson Symphony Center, we ask that each attendee sign this form to ensure health and safety of the event.
